Job description: Home Health Intake Nurse (RN or LPN)
Are you looking for a dynamic environment where you can maximize your home health knowledge? Hillcrest Home Care, a locally owned healthcare provider focused on aging adults, has an opportunity for a Home Health Intake Nurse/Care Navigator (RN or LPN).
This team member engages with referral partners and provides a clinical review and handoff of incoming clients, with the goal of connecting clients with needed home health services.
The ideal candidate has the ability to work in an independent setting, applying clinical judgment and critical thinking to the intake process. Previous home health experience (field or office) is required. This is a Mon-Fri daytime office position, with future remote work options for qualified candidates.
A Home Health Intake Nurse/Care Navigator promotes the growth of Hillcrest Home Care by representing the organization, its mission, and its role in the community in a professional manner.

Requirements:
A graduate of an accredited nursing program
Active license to practice as an LPN or RN in the state of Nebraska or another nurse licensure compact state
Minimum two years nursing experience, including acute and post-acute services
Home health clinical or referral management experience preferred
Business development/marketing experience preferred
Ability to solve practical problems and deal with a variety of standards effectively
Proficient at basic computer/laptop, such as opening and closing Windows applications.
