Provide personalized and high-quality care to patients in their homes, following the established protocols and guidelines of Allina Health System. Assist patients with daily living activities such as bathing, dressing, and grooming, as well as housekeeping tasks. Monitor patient's health status and report any changes or concerns to the appropriate healthcare professionals. Administer medications and treatments as prescribed by the patient's physician, while adhering to all safety protocols and procedures. Maintain accurate and detailed patient records, including daily progress notes, medication administration logs, and any other required documentation. Communicate effectively with patients and their families, providing support, education, and guidance as needed. Collaborate with other healthcare team members to develop and implement individualized care plans for each patient. Follow infection control policies and procedures to ensure a safe and healthy environment for patients. Attend regular training and education sessions to stay updated on best practices and new developments in the home care field. Adhere to all ethical and legal standards, maintaining patient confidentiality and respecting their rights and dignity at all times. Participate in regular performance evaluations and provide feedback for continuous improvement of the home care services provided by Allina Health System. Maintain a positive and professional attitude, promoting a welcoming and supportive environment for patients, their families, and team members.
