[Hiring] RN Navigator, Bone and Joint @American Addiction Centers
Role Description
This is a remote position. Secondary location is the NEW ASC/MOB Greensboro on Horse Pen Road.
ā¢ Utilizes the nursing process to provide patient/family centered care focusing on the physical, emotional, spiritual, cultural, religious, and environmental needs.
ā¢ Coordinates the effective and efficient delivery of patient care services through the application of care management principles.
ā¢ Acts as a patient advocate throughout the continuum of care including all patient settings and in the community.
ā¢ Documents in accordance with departmental reporting standards.
ā¢ Implements outreach efforts to establish and maintain positive working relationships with key community agencies.
ā¢ Demonstrates the ability to function in a professional setting through active participation in a professional practice model.
ā¢ Communicates effectively to promote sharing of information throughout the continuum of care.
ā¢ Responsible for ongoing evaluation and quality improvement of the Navigator Program.
ā¢ Works closely with community based service providers to identify and address community needs for services and education.
ā¢ Provides age/developmentally appropriate patient care in accordance with Age-Specific guidelines.
Qualifications
ā¢ Graduate from an accredited school of nursing, BSN required.
ā¢ Minimum two years' experience as a registered nurse in the specialty required.
Requirements
ā¢ Current licensure to practice as Registered Nurse in applicable state.
ā¢ Certification as an RN preferred.
ā¢ BLS required.
ā¢ Requires problem solving, decision making and critical thinking.
ā¢ Ability to work in a self-directed environment and with leading teams.
ā¢ Demonstrates ability to work in fast-paced environment with multiple interruptions.
ā¢ Expertise in MS Office.
ā¢ Ability to prepare program reports frequently and efficiently.
ā¢ Bilingual preference with fluency in Spanish desirable.
