Role Description
The Population Health Pharmacy Technician I for Stellus Rx will help our communities thrive as a key member of the Adhere Team. You will work closely with Stellus Rx leaders and across the organization, as we work collaboratively to unlock the health of millions of Americans by turning "use as prescribed" into a guarantee, not a direction.
ā¢ Support medication reconciliation processes for patients within the designated population.
ā¢ Collaborate with pharmacists and healthcare providers to optimize medication regimens.
ā¢ Review patient profiles to identify potential medication-related issues and discrepancies.
ā¢ Collect and analyze medication-related data to identify trends and opportunities for improvement.
ā¢ Generate reports on medication adherence, therapeutic outcomes, and other relevant metrics.
ā¢ Work closely with healthcare teams to ensure seamless coordination of pharmaceutical services.
ā¢ Communicate with patients, caregivers, and healthcare professionals to address medication-related concerns.
ā¢ Participate in population health programs aimed at promoting medication adherence and preventive care.
ā¢ Support initiatives focused on improving health outcomes and reducing medication-related disparities.
ā¢ Ensure compliance with regulatory standards, pharmacy laws, and organizational policies.
ā¢ Contribute to quality improvement efforts within the pharmacy setting.
Qualifications
ā¢ High school diploma or equivalent; completion of a pharmacy technician program preferred.
ā¢ Certification or licensure as a pharmacy technician (requirements may vary by location).
ā¢ Strong understanding of pharmacy operations, medication management, and healthcare systems.
ā¢ Excellent communication, interpersonal, and organizational skills.
ā¢ Proficiency in using pharmacy software and computer systems for data entry and analysis.
ā¢ Ability to work collaboratively in a multidisciplinary team environment.
ā¢ Ability to speak fluent Vietnamese.
