[Hiring] Clinical - CCM LVN @Nsight Health
Role Description
This role involves providing virtual patient care through chronic care management.
ā¢ Conduct thorough assessment calls with patients to gather relevant information about their health status, concerns, and needs
ā¢ Identify and escalate patient needs, including refill requests, appointment scheduling, and connecting patients with community resources
ā¢ Provide patients with relevant and clear information about their health conditions, treatment plans, and self-management strategies
ā¢ Collaborate with patients to establish realistic and achievable health goals, aligning with their overall care plans
Qualifications
ā¢ Active LVN/LPN license (required)
ā¢ At least 1 year of nursing experience preferred (RPM, telehealth, or chronic care experience a plus)
ā¢ Strong communication skills with the ability to build patient trust remotely
ā¢ Proficient with computers (Mac), EMRs, and telehealth tools
ā¢ Reliable, high-speed internet and a HIPAA-compliant home workspace
ā¢ A hardwired internet connection is required to ensure call clarity
Requirements
ā¢ Schedule: Monday - Friday 9:00 am - 6:00 pm EST
