Position Summary:
At the 24-Hour Nurse Line the Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members decisions related to achieving and maintaining optimal health status. Requires an RN with unrestricted active license.
Hours required: Start time will be 9 am EST, 8 hour shift, every other weekend, 4 holidays per year.
Must reside in a nurse Compact State and hold a Multi State Compact license.
The 24-Hour Nurse Line provides a telephone-based information and decision support program to eligible members. The Nurse will be responsible for the following:
Facilitates effective health care decision-making.
Improves the quality of care.
Improves member satisfaction with their health care plan.
Facilitates appropriate use of health care resources.
Provides up-to-date health care information to help facilitate members understanding of their health problems and possible options.
Provides assistance by clarifying available options for the member's particular health problem or issue.
Helps members actively and knowledgeably participate with their provider in their own health care decision-making.
Promotes healthy lifestyles to help lower personal health risks, preventing illness.
Awareness of the members available health and wellness programs, and makes referrals to these programs as appropriate.
Required Qualifications:
Active unrestricted RN compact state licensure.
3+ years of clinical experience required, with preference for the following areas: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology.
1 year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word).
Must be open to a start time of 9 am EST, 8 hour shift, every other weekend, 4 holidays per year.
Willingness to obtain additional state licenses upon hire
Preferred Qualifications:
Managed Care experience preferred.
Previous coaching experience preferred.
Model candidates have telephone triage nursing experience.
Education:
Associate Degree minimum required.
Bachelor's Degree preferred.

Pay Range
The typical pay range for this role is:
$54,095.00 - $116,760.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
