As a Field HCBS Wait List Care Coordinator, you will be responsible for developing and implementing person-centered, goal-oriented care plans that address the unique needs of individuals receiving home and community-based services. Your expertise will ensure that our clients receive timely, effective, and cost-efficient care, while also promoting their independence and well-being.

Flexible scheduling, with a standard Monday-Friday schedule between 8am-5pm
Local travel required (up to 75%), with mileage reimbursement at the current government rate

Primary Responsibilities:

Assess, plan, and implement individualized care strategies that promote the most appropriate, least restrictive level of care
Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
Ensure care plans address social determinants of health, covered services, and value-added benefits
Communicate effectively with stakeholders to ensure quality, coordinated care and services
Advocate for individuals and families, ensuring their needs and choices are fully represented and supported
Conduct home visits, in-person visits, and serve as a resource for community care coordinators

Required Qualifications:

Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services field
1+ years of experience working with individuals with long-term care needs
1+ years of experience working in a community health setting in a healthcare role
Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications
Ability to travel 75% of the time, with a valid driver's license, current automobile insurance, and reliable transportation
Access to a designated quiet workspace in your home, with a secure internet connection

Preferred Qualifications:

Licensed Social Worker or clinical degree
Experience working with individuals receiving services on one of the home and community-based waivers in Kansas
Background in managing populations with complex medical or behavioral needs
Experience with electronic charting and arranging community resources

Soft Skills:

Strong communication and customer service skills
Ability to work independently, with good judgment and accountability
Demonstrated ability to work well with others, with strong organizational and time management skills
Ability to multi-task, prioritize tasks, and meet deadlines in a fast-paced environment
