Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities.

• Assess, plan and implement care strategies for transition that are individualized by a patient and directed toward the most appropriate, lease restrictive level of care.

Primary Responsibilities:
• Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services
• Manage the care plan throughout the transition between groups process as a single point of contact
• Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
• Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team
• Advocate for transitions of care for those individuals institutionalized long term to return to the community for their care
• Coordinate multiple resources and providers to develop a solid person centered support plan for care in the community
• Ability to communicate effectively with Behavioral Health Teams, Provider Relations team, Member Advocacy team, Long Term Service s and Supports Field Staff and their managers to provide the highest quality support plans for CHOICES Individuals

Required Qualifications:
• RN or Social Worker license in the state of TN or clinical degree
• 5+ years of experience working with MS Word, Excel and Outlook
• 3+ years of experience working within the community health setting in a health care role
• Experience in long-term care, home health, hospice, public health or assisted living
• Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
