Assess, plan and implement care strategies for transition that are individualized by a patient and directed toward the most appropriate, lease restrictive level of care.

Primary Responsibilities:
Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services
Manage the care plan throughout the transition between groups process as a single point of contact
Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
Advocate for patients and families as needed to ensure the patients needs and choices are fully represented and supported by the health care team
Advocate for transitions of care for those individuals institutionalized long term to return to the community for their care
Coordinate multiple resources and providers to develop a solid person centered support plan for care in the community
Ability to communicate effectively with Behavioral Health Teams, Provider Relations team, Member Advocacy team, Long Term Service s and Supports Field Staff and their managers to provide the highest quality support plans for CHOICES Individuals

Required Qualifications:
RN or Social Worker license in the state of TN or clinical degree
5+ years of experience working with MS Word, Excel and Outlook
3+ years of experience working within the community health setting in a health care role
Experience in long-term care, home health, hospice, public health or assisted living
Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
