Field Case Manager RN - South Cook County

Conduct comprehensive assessments of patients' medical needs and develop individualized care plans. Coordinate and collaborate with healthcare professionals to ensure continuity of care and appropriate treatment plans. Provide ongoing monitoring and evaluation of patients' progress and adjust care plans as needed. Educate patients and their families on disease management, medication management, and self-care techniques. Advocate for patients' needs and ensure they receive appropriate medical services and resources. Maintain accurate and up-to-date documentation of patient interactions and care plans. Communicate effectively with patients, families, and healthcare team members to facilitate optimal care. Utilize critical thinking skills to identify and address potential barriers to care and develop solutions. Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving outcomes. Stay current on healthcare industry trends and best practices in case management. Adhere to all regulatory and compliance standards. Represent Aetna and maintain a positive and professional image in the community. Travel to various locations within South Cook County to provide in-person case management services. Participate in training and continuing education opportunities to enhance skills and knowledge.
