Field Case Manager - Registered Nurse

Position Summary This role serves the Northern Virginia area, and qualified candidates are required to live in Reston, Fairfax County, or surrounding areas. Work Schedule: Monday-Friday during standard business hours. Roles and Responsibilities As a Nurse Case Manager, you will be responsible for both telephonic and face-to-face assessments, planning, implementing, and coordinating all case management activities with members. Your primary focus will be to evaluate medical needs and enhance the overall wellness of our members. You will: • Develop proactive strategies to address health concerns and improve outcomes. • Utilize clinical tools and data to evaluate member needs and facilitate transitions to Aetna programs. • Apply clinical judgment to reduce risk factors and address complex health indicators. • Conduct thorough assessments that include consideration of co-morbid conditions impacting functionality. • Engage in case conferences for a multidisciplinary approach to case management. • Utilize interviewing skills to maximize member engagement and ascertain health needs. Required Qualifications • Current unrestricted RN licensure in Virginia. • 3+ years of clinical experience as a Registered Nurse (med-surg, behavioral health, acute care). • Must reside in Reston, Virginia, or surrounding areas. • Willingness to travel up to 75% of the time to meet members in their homes. Preferred Qualifications • Experience in case management within an integrated model. • Bilingual proficiency in English and Spanish is highly desirable. • Strong multitasking and prioritization skills in a fast-paced environment. • Proficient in navigating multiple computer systems. • Excellent verbal and written communication skills. Education • Associate degree in Nursing required; Bachelor's degree preferred. • Unrestricted Virginia State RN License required. Anticipated Weekly Hours 40 Time Type Full time Pay Range Salary range: $60,522.00 - $129,615.00. Actual salary offers will depend on various factors including experience and education. This position is also eligible for a bonus or incentive program in addition to base pay.
