Field Care Navigator
Care Navigator – Home-Based Senior Care Brigade Health Full-Time | Monday–Friday At Brigade Health, we’re redefining how seniors receive care—bringing compassion, connection, and clinical excellence directly to where they live and thrive. We’re seeking a Care Navigator to join our mission-driven team supporting seniors in their homes, senior living, and memory care communities. This role is ideal for someone who enjoys variety, values relationships, and finds meaning in helping others. What You’ll Do: • Visit patients in their residential homes to support care plans • Create individualized care plans to meet the needs of each patient • Assist with dementia program activities and telehealth visits • Provide care coordination and resource support • Build relationships with patients, families, and community partners • Support documentation, scheduling, and community outreach events What We’re Looking For: • Compassionate, adaptable, and reliable team player • Comfortable working independently and in diverse environments • Medical Assistant diploma preferred • Experience or education in patient care or case management • Valid driver’s license and ability to travel locally Join a team that values connection, dignity, and whole-person wellness.
