Provide exceptional care and support to members in accordance with Aetna's standards and policies. Promote health and wellness by educating and empowering members to make informed decisions about their health. Advocate for members' needs and collaborate with healthcare providers to ensure their medical needs are met. Conduct thorough assessments of members' health and identify any potential barriers to care. Develop individualized care plans that address members' physical, emotional, and social needs. Monitor and evaluate members' progress towards health goals and make necessary adjustments to their care plans. Communicate with members, families, and healthcare providers to ensure continuity and coordination of care. Keep accurate and up-to-date documentation of member interactions, care plans, and progress. Stay current on industry trends and best practices in care management, and apply them to improve the quality of care for members. Collaborate with other members of the Aetna team to ensure seamless and efficient delivery of care. Maintain confidentiality and adhere to all HIPAA and privacy regulations. Act as a resource and support system for members, addressing any concerns or questions they may have. Participate in training and development opportunities to continuously enhance skills and knowledge. Ensure compliance with all regulatory requirements and company policies.
