Responsibilities
Assess and evaluate member's needs to achieve optimal wellness.

Guide members and families towards appropriate resources for care.

Perform telephonic and face-to-face assessments of members.

Develop individualized care plans based on clinical judgment and member needs.

Monitor member progress towards desired health outcomes.

Collaborate with providers and community services for quality care.

Coordinate delivery of services for physical health and social determinants of health.

Submit incident reports as necessary.
Requirements
Unrestricted Licensed Registered Nurse in the State of Louisiana.
,
2 years of experience in nursing or healthcare field (discharge planning, case management, care coordination, or home/community health).
,
Extensive nursing experience across all ages.
,
Knowledge of community health and social service agencies.
,
Intermediate to advanced computer skills, including Microsoft Word, Outlook, and Excel.
,
Ability to use electronic medical records and various software applications.
,
Exceptional communication and interpersonal skills.
,
Ability to work with minimal supervision and a flexible schedule.
Nice-to-haves
BSN (Bachelor of Science in Nursing).

3-5 years of in-home assessment and care coordination experience.
,
Experience with health promotion, coaching, and wellness.
,
Previous managed care experience.
,
Bilingual in English and Spanish.
,
Certification in Case Management.
