Key Responsibilities In this role, you will be responsible for: Researching available options and resources for members with financial, transportation, safety, and housing needs, spanning the entire lifecycle. Empowering members to make positive lifestyle and medical choices through telephonic or electronic exchanges. Working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health. Utilizing your expertise to navigate complex cases and provide support to members in need. Maintaining accurate and detailed records of member interactions and interventions. Essential Qualifications To be successful in this role, you must possess: An active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in your state of residence. A minimum of 2+ years of experience in medical social work or case management. A Master's Degree in Social Work. Preferred Qualifications While not required, the following qualifications are preferred: Experience working in a telephonic or remote case management setting. Knowledge of community resources and ability to navigate complex systems. Strong communication and interpersonal skills, with the ability to work effectively with members, families, and healthcare teams.
