Position Overview We're looking for a skilled Case Manager RN to work in the field, serving members in Wayne and Macomb Counties. As a Case Manager, you'll play a vital role in assessing, planning, implementing, and coordinating care management activities to address the complex health and social needs of our members. This is a unique opportunity to work independently from home, with 25-50% travel within the designated counties. Key Responsibilities: Conduct comprehensive assessments of members' needs, using clinical tools and data review to identify areas for improvement. Develop proactive plans of care to address identified issues, enhancing short-term and long-term outcomes, and promoting overall wellness. Apply clinical judgment to incorporate strategies that reduce risk factors and barriers, addressing complex health and social indicators that impact care planning. Collaborate with supervisors, providers, and community organizations to overcome barriers, meet goals, and present cases at interdisciplinary case conferences. Utilize case management processes in compliance with regulatory and company policies, and motivational interviewing skills to ensure maximum member engagement. Maintain accurate and up-to-date records, using effective computer skills and software applications. Requirements: Active Michigan state license as a Registered Nurse. Minimum 3+ years of clinical experience, with 2+ years in care management, discharge planning, or home health care coordination. Ability to travel 25-50% within Wayne and Macomb Counties. Associates Degree or Nursing Diploma required; BSN preferred. Preferred Qualifications: Experience working with Medicaid or waiver populations. Bilingual skills in Arabic or Spanish. Certified Case Manager (CCM) or equivalent certification. Excellent analytical, problem-solving, and communication skills. Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint.
