Job Overview As a Primary Care Provider, you will play a vital role in our healthcare team, working closely with physicians and other healthcare professionals to provide comprehensive care to our patients. Your expertise will be instrumental in improving patient outcomes, promoting health and wellness, and addressing the complex needs of our patient population. Key Responsibilities Conduct comprehensive assessments to identify patient needs and develop effective care plans. Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions. Provide care during short-term sub-acute rehabilitation episodes, ensuring seamless transitions and optimal patient outcomes. Maintain accurate and complete documentation, including ICD-10 conditions, to ensure continuity of care. Communicate and collaborate with the interdisciplinary care team to ensure comprehensive and coordinated care. Engage in advanced illness and advanced care planning conversations with patients and their caregivers. Offer counseling and education to patients and caregivers, empowering them to make informed decisions about their care. Essential Qualifications To be considered for this role, you must possess the following essential qualifications: A Master's or Doctoral degree in Nursing from an accredited program. An active and unrestricted license to practice in the state where you reside, or the ability to obtain one by the start date. Current, active DEA licensure/prescriptive authority, or the ability to obtain it post-hire, in accordance with state regulations. The ability to establish a collaborative practice agreement, if required by your state. The physical capability to perform the job requirements, including lifting a 30-pound bag, navigating stairs, and sitting, standing, and kneeling to conduct physical assessments. Access to reliable transportation to facilitate remote work. Preferred Qualifications While not mandatory, the following qualifications are highly desirable: A minimum of 1 year of clinical experience in a practice setting, preferably in long-term care. Experience in addressing the complex medical needs of patients with behavioral, social, and/or functional challenges. A strong understanding of geriatrics and chronic illness management. Knowledge of advanced illness and end-of-life care discussions. Proficiency in computer skills, including electronic medical records and written documentation.
