Job Overview We are seeking an experienced and compassionate Hospice Case Manager - RN to join our team in Port Arthur, TX. As a Hospice Case Manager, you will be responsible for delivering professional nursing care to our patients in accordance with the established plan of care. You will lead by example, providing quality care consistent with all relevant professional standards and state and federal requirements. Key Responsibilities Deliver professional nursing care to patients in accordance with the established plan of care Complete comprehensive subjective and objective assessments of patient status, including physical, psychosocial, and environmental parameters Formulate individualized plans of care that incorporate analysis of assessment data and modify and update as needed Provide skilled interventions aimed at achieving realistic outcomes within a specified time period Assure thorough and accurate documentation with all clinical data, including admission through discharge, physician's orders, skilled nursing visits, and changes in conditions Consistently demonstrate competency with technical nursing skills according to the legal scope of practice Implement teaching specific to patient/family needs and evaluate and modify as needed Perform clinical chart record reviews as requested and perform on-call responsibilities as assigned Essential Qualifications Current RN licensure in the state of Texas Minimum 2 years of experience in hospice care or a related field Strong knowledge of hospice regulations and standards of practice Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills Preferred Qualifications Bachelor's degree in Nursing or a related field Certification in Hospice and Palliative Care (CHPLN or CHPN) Experience with electronic medical records and documentation systems Strong leadership and mentoring skills Skills and Competencies Strong clinical skills and knowledge of hospice care Excellent assessment and planning skills Ability to work in a fast-paced environment and prioritize multiple tasks Strong communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills Career Growth Opportunities and Learning Benefits At Intrepid USA, we are committed to the growth and development of our team members.
