Job Overview We are seeking a highly organized and detail-oriented individual to join our team as a Home Health Intake Coordinator. As an Intake Coordinator, you will play a vital role in ensuring the smooth and efficient operation of our home health agency. Your primary responsibility will be to receive referrals, communicate with hospital case managers, staff clinicians, and coordinate with the office staff for the overall care of our patients. This is a fully remote position, and we are looking for someone who can stay in this position for at least a couple of years. Duties and Responsibilities As a Home Health Intake Coordinator, your key responsibilities will include: Reviewing referrals from hospitals, ensuring accurate demographic and insurance information is obtained Scheduling clinicians and coordinating with all staff to ensure timely initiation of care Collecting and verifying patient information, including medical history and insurance coverage Assisting with medical records management, including scanning, filing, and retrieval Utilizing electronic health record systems to input and update patient data Answering phone calls and responding to inquiries in a professional and courteous manner Collaborating with other departments to ensure seamless coordination of patient care Providing administrative support to medical staff as needed Qualifications and Requirements To be successful in this role, you will need: Previous experience working in a medical office or healthcare setting (preferred) Familiarity with front desk operations, including appointment scheduling and patient check-in processes Knowledge of medical terminology and procedures Proficient in using electronic health record systems Strong organizational skills with the ability to multitask and prioritize workload effectively Excellent communication skills, both written and verbal Familiarity with phone systems and returning voicemails Remote work experience (preferred) Skills and Competencies To excel in this role, you will need to possess: Strong attention to detail and accuracy Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills Proficiency in using electronic health record systems and other software applications Ability to learn and adapt to new systems and processes Career Growth Opportunities and Learning Benefits At Workwarp, we are committed to the growth and development of our team members.
