We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team as a Care Coordinator. Job Summary

We are looking for a highly skilled and motivated LPN/LVN to work as a Care Coordinator, providing chronic care management services to Medicare patients with multiple chronic conditions. As a Care Coordinator, you will work closely with Physicians/Practitioners, patients, and their families to develop and implement personalized care plans. This is a full-time remote opportunity, with set hours from 8:30 am CST to 5:00 pm MST. We have three positions available for June and July start dates.

Key Responsibilities


Assist Physicians/Practitioners in managing the care of Medicare patients with chronic conditions
Develop and implement personalized care plans for patients, ensuring a minimum of 20 minutes of care coordination per patient per month
Conduct wellness checks, provide education on conditions, and assist with appointments, prescription refills, and referrals
Manage a caseload of 12-14 patients daily, working independently in a remote setting
Excellent phone skills, with the ability to communicate effectively with patients, caregivers, and healthcare providers
Superior time management and communication skills, with the ability to prioritize tasks and meet deadlines
Document all activities in the Electronic Health Record (EHR) and submit daily reports
Identify trends and opportunities for improvement, and escalate patients who require reassessment to the Care Manager


Essential Qualifications


Current LPN or LVN licensure in the United States
At least 1 year of experience working with Medicare patients with multiple chronic conditions
Proficient in Microsoft Office (Excel, Word) and EHR systems
Strong multi-tasking and organizational skills, with the ability to navigate multiple platforms simultaneously
Positive attitude, with excellent communication and interpersonal skills
Self-motivated and able to work independently in a remote setting


Preferred Qualifications


Experience with chronic care management services and Medicare regulations
Familiarity with care coordination and case management principles
Certification in care coordination or a related field (e.g., CCM, CRC)


Skills and Competencies


Strong clinical knowledge of chronic conditions and their management
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Strong organizational and time management skills
Proficient in EHR systems and Microsoft Office
