Job Summary We're seeking an experienced Full Stack Virtual Primary Care Provider – Western Region, Family Nurse Practitioner to join our National Virtual Primary Care team. As a key member of our collaborative team, you'll work in a remote office, providing high-quality, patient-centered care to patients 18 years of age and older. You'll be responsible for assessing, diagnosing, treating, and managing health problems, as well as providing health counseling and disposition planning. Key Responsibilities Provide evidenced-based, primary care to patients 18 years of age and older, including accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. Collaborate with a dedicated care team to ensure the delivery of high-quality, patient-centric, longitudinal care. Participate in daily care team huddles to review patient care, including pre-charting, open care gaps, labs/diagnostic reports, and referrals. Communicate effectively with members of the care team, other health professionals, and health-related agencies. Work effectively with care team members, promoting mutual respect and trust within a team model. Cultivate and maintain positive relationships with care team members, patients, and families. Manage clinical and non-clinical duties efficiently, adapting quickly to new models of patient care for clinic efficiency. Demonstrate core values of CVS Health in all communications and interactions, enhancing operational effectiveness while emphasizing cost containment without jeopardizing innovation or quality of care. Participate in professional development through organizational and departmental meetings, educational opportunities, committees, and workgroups as necessary. Essential Qualifications Completion of Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role. Current National Board Certification, Family Practice. A minimum of 5 years of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice. Effective verbal, written, and electronic communication skills. Outstanding organizational skills and ability to multi-task. Initiative, problem-solving ability, adaptability, and flexibility. Ability to work remotely in a care team culture. Proficient with information management and technology. Ability to obtain and maintain multi-state licensures. Preferred Qualifications Experience working in a virtual care environment. Knowledge of quality measurement in the management of chronic disease conditions. Experience with electronic health records and practice management systems. Leadership experience in a healthcare setting.
