Job Summary As a Data Entry Assistant at blithequark, you will serve as an administrative support for our field care teams, ensuring the accuracy and completeness of medical records. You will review medical records, code clinical data using standard classification systems, and coordinate medical record transmission with provider offices, health system partners, and other clinical entities. This is a full-time remote opportunity that requires strong organization skills, attention to detail, and proficiency with Microsoft Office Suite. Key Responsibilities Medical Record Review and Management : Review medical records for accuracy and completeness, and ensure that all patient information is stored and transmitted in compliance with company privacy policies and HIPAA. Chart Requests and Processing : Verify and process requests for charts to be pulled for patient care, quality review, and audits in a timely manner. Medical Record Scanning and Indexing : Accurately scan and index medical records to the appropriate chart. Communication and Collaboration : Process and manage inbound and outbound communications in a professional manner, and establish positive relationships with providers and patients. Data Entry and Verification : Enter, review, and verify member and provider information within the care management platform. Administrative Duties : Complete administrative duties related to patient and provider care plan delivery, and communicate with care teams regarding admission and discharge status of members. Medical Record Retrieval : Obtain pertinent medical records from providers (hospitals, nephrology, home health agencies) and manage scan information within the care management platform. Meeting Attendance : Attend meetings as requested. Additional Responsibilities : Perform other duties and responsibilities as required, assigned, or requested. Requirements and Qualifications Essential Qualifications Education : High school diploma or GED required. Experience : At least one year of medical records experience working in a healthcare setting. Computer Skills : Basic computer skills, including the ability to scan, organize, and access electronic health records. Data Entry Skills : Strong data entry skills with keen attention to detail to ensure accuracy. Organization and Time Management : Advanced organization skills and excellent time management skills. Microsoft Office Suite : Experience using Microsoft Office Suite. Preferred Qualifications Clinical Knowledge : Familiarity with clinical terminology and coding systems. Electronic Health Records : Experience with electronic health records (EHRs) and practice management systems. Communication Skills : Excellent written and verbal communication skills.
