Job Summary

We're seeking an experienced Field RN Care Coordinator to join our team in Dallas and Tarrant County, TX. As a Field RN Care Coordinator, you will play an essential role in our Integrated Care Model by relaying pertinent information about member needs and advocating for the best possible care available. You will ensure that our members have the right services to meet their needs, working closely with nursing facilities and other stakeholders.

About the Role

This is a Field-based position with a home-based office, requiring travel to local areas about 75% of the time based on business needs. If you are within a 30 to 60 mile radius of Dallas and Tarrant Counties, TX, and can travel locally, you will have the flexibility to work remotely. This role is ideal for a motivated and compassionate RN who is passionate about delivering high-quality care and making a meaningful difference in the lives of our members.

Primary Responsibilities

As a Field RN Care Coordinator, your primary responsibilities will include:


Assess, plan, and implement care strategies that are individualized by member and directed toward the most appropriate, least restrictive level of care.
Identify and initiate referrals for member transition to community resources and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services.
Manage the care plan throughout the continuum of care as a single point of contact.
Communicate with all stakeholders for the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the healthcare team.
Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices.

Required Qualifications

To be successful in this role, you will need to have:


A current, unrestricted RN license in the state of Texas
2+ years of experience working within the community health setting or in a healthcare role
Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications
Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings
Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
Reside in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service


Preferred Qualifications

While not required, we prefer candidates with:


1+ years of experience with long-term care services and support, Medicaid or Medicare
Ability to create, edit, save, and send documents, spreadsheets, and emails
Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
Reside within commutable distance of assigned duties
