Job Overview We are seeking an experienced Field Health Social Services Coordinator to join our team in North Johnson County, TX. As a Field Health Social Services Coordinator, you will play a critical role in assessing, planning, and implementing care strategies that are individualized to each patient's needs. You will work closely with patients, families, and healthcare providers to ensure that patients receive the most appropriate and least restrictive level of care. This is a remote-based position that offers flexibility and autonomy, allowing you to work from the comfort of your own home while making a meaningful difference in the lives of others. Primary Responsibilities Assess, plan, and implement care strategies that are individualized to each patient's needs and directed towards the most appropriate, least restrictive level of care Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services Manage the care plan throughout the continuum of care as a single point of contact, communicating with all stakeholders to ensure quality, coordinated care and services are provided expeditiously to all members Advocate for patients and families as needed to ensure that the patient's needs and choices are fully represented and supported by the healthcare team Why Join UnitedHealth Group?
