Job Summary

As a key member of our telehealth team, you'll be responsible for providing exceptional patient care, coaching, and education to individuals managing chronic conditions. Responsibilities
Triage patient calls and provide timely responses to their inquiries and concerns.
Conduct regular condition assessments to identify areas for improvement and develop personalized care plans.
Collaborate with patients to set and achieve health goals, providing education and coaching on disease management and self-care.
Perform monthly medication reconciliations to ensure accurate and up-to-date medication lists.
Develop and implement patient-centered care plans that address their unique needs and goals.
Communicate effectively with healthcare providers to ensure seamless care coordination and transitions.
Provide education and support to patients on disease management, self-care, and healthy lifestyle habits.
Qualifications
To be successful in this role, you'll need to possess the following qualifications and skills:
CMA or RMA certification (required)
Prior experience in Chronic Care Management (CCM) or Remote Patient Monitoring (RPM) (preferred)
Bilingual Spanish (preferred)
Strong knowledge of third-party payer requirements and electronic medical records
Excellent organizational, problem-solving, communication, and interpersonal skills
Ability to work independently and as part of a remote team
Strong attention to detail and ability to maintain accurate records
Preferred Qualifications
We're looking for candidates with the following preferred qualifications:
1+ year of experience in RPM or CCM
1+ year of experience in telehealth
Certified Medical Assistant Certificate (CMA or RMA)
Bilingual Spanish

Compensation and Benefits
We offer a competitive hourly rate of $18.00 - $21.00 per hour, depending on experience. As a 1099 contractor, you'll have the flexibility to work as many or as few hours as you'd like, with a minimum of 30 hours per week. We also offer a flexible schedule, with day shifts available.
