**Job Summary**

As an Experienced Case Management Specialist, you will be responsible for assessing the needs of our DSNP members, developing and implementing individualized care plans, and coordinating with healthcare providers and community resources to ensure seamless care delivery. You will work closely with our multidisciplinary team to identify and address high-risk factors, provide education and support to members, and promote healthy behaviors and lifestyle changes.

**Key Responsibilities**

* Conduct comprehensive assessments of DSNP members' needs, using care management instruments and data analysis to identify areas for improvement and develop effective care plans.
* Recognize high-risk factors and service needs that may impact member outcomes, and collaborate with healthcare providers and community resources to address these needs.
* Develop and implement individualized care plans, working closely with members, families, and healthcare providers to ensure seamless care delivery.
* Utilize case management and quality management processes to ensure compliance with regulatory and licensure requirements, as well as company policies and procedures.
* Provide education, support, and advocacy to members and their families, empowering them to make informed decisions about their care and promoting healthy behaviors and lifestyle changes.
* Collaborate with healthcare providers and community resources to ensure timely and effective care delivery, and to address any barriers to care.
* Document all interactions with members, families, and healthcare providers, using electronic health records and other documentation tools as required.

**Essential Qualifications**

* 3+ years of experience in a health-related field, with a strong understanding of case management principles and practices.
* Bachelor's degree or equivalent experience required.
* Strong assessment and problem-solving skills, with the ability to analyze complex data and develop effective care plans.
* Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations, including individuals with complex health and social needs.
* Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.

**Preferred Qualifications**

* Certification in case management, such as CRC, CDMS, CRRN, COHN, or CCM.
* Experience working with geriatric populations, social health, and disability populations.
* Bilingual or multilingual skills, with the ability to communicate effectively with diverse populations.
* Experience working with federal health insurance and Medicaid programs.
