About the Role: Analyst - Care Planner As an Analyst - Care Planner at CVS Health, you'll be responsible for analyzing patient care plans and health risks to ensure continuity and effectiveness. This involves collaborating with healthcare teams to identify patient needs, developing and implementing care plans, and monitoring patient progress. Your expertise will be crucial in driving patient-centered care, improving health outcomes, and reducing healthcare costs. Key Responsibilities: Analyze patient care plans and health risks to identify areas for improvement and optimize patient outcomes. Develop and implement care plans tailored to individual patient needs, taking into account their unique health risks and circumstances. Collaborate with healthcare teams to ensure seamless care coordination, sharing insights and expertise to drive patient-centered care. Monitor patient care plans and present updates to healthcare teams, highlighting progress, challenges, and areas for improvement. Maintain accurate records of patient care plans and health risks, ensuring data integrity and confidentiality. Provide support and education to patients and caregivers on care plans, empowering them to take an active role in their healthcare journey. Stay up-to-date on the latest healthcare trends and best practices , applying this knowledge to inform care planning decisions and drive continuous improvement. Address issues and concerns related to patient care plans, utilizing problem-solving skills to identify creative solutions. Ensure compliance with relevant healthcare regulations and guidelines, maintaining the highest standards of quality and integrity. Essential Qualifications: To succeed in this role, you'll need: A minimum of two years of experience as an Analyst - Care Planner or in a related role, with a proven track record of analyzing patient care plans and developing targeted interventions. Excellent communication, organizational, and analytical skills, with the ability to distill complex data into actionable insights. A strong understanding of healthcare operations, including care planning, disease management, and health risk assessment. A Bachelor's Degree in Health Science or a related field is preferred, although not required.
