Assisting with patient care coordination, including scheduling appointments and follow-up visits

Essential Qualifications
To be successful in this role, you will need:

A minimum of 2 years of medically relevant experience or equivalent
Completion of the Dot certification test within 30 days of employment
Basic Life Support (BLS) certification, including mouth-to-mouth and Automated External Defibrillator (AED) training
Ability to pass a FIT test and wear a respirator mask when required
Strong communication and interpersonal skills, with the ability to work effectively with patients, healthcare professionals, and administrative staff
Proficiency in electronic medical records (EMRs) and other healthcare software applications


Preferred Qualifications
While not required, the following qualifications are preferred:

Previous experience as a licensed provider, with a graduate degree-level Family Nurse Practitioner program or equivalent
Current National Board Certification and state licensure to practice in the Advanced Practice Registered Nurse (APRN) role
Proficiency in additional languages, including Spanish
Experience with patient education and health promotion, including health screenings, social determinants of health, and chronic disease management
