Make outbound calls to assess members' current health status
Identify gaps or barriers in care and treatment plans
Make referrals to outside resources
Facilitate a continuum of quality care through close communication with members and/or caregivers via n-phone interaction
Support members with condition education, medication reviews, and connections to agencies and partners in the community such as Home Health Aides or Meals on Wheels
Respond to designated inquiries from the Health Care Authority by contacting member, providers, and various internal teams to resolve the issue
Assist with Fair Hearings by reviewing member history, contacting member to preemptively resolve the issue before the hearing if possible, collaborating with Fair Hearing team, attending Fair Hearing meetings
Other duties as assigned
This is a high volume, customer service environment. You’ll need to be efficient, productive, and thorough, in dealing with our members, providers and outside resources over the phone. Solid computer and software navigation skills are critical. You should also be solidly patient-focused and adaptable to changes.You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications
Current, unrestricted RN license in the state of employment
Ability to work in Pacific Standard Time
2+ years of experience in a hospital setting, acute care, direct care experience, or experience as a telephonic Case Manager for a managed care organization
Demonstrated ability to type and have the ability to navigate a Windows based environment
Preferred Qualifications
BSN
Residency in the state of Washington
Certified Case Manager (CCM)
Case management experience
Experience or exposure to discharge planning
Experience in utilization review, concurrent review or risk management
Experience in a telephonic role
Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
Background in managed care
Bilingual or multilingual skills
Demonstrated ability to establish rapport quickly and willingness to collaborate to problem-solve
Demonstrated ability to communicate effectively and tactfully both verbally and in writing.
Demonstrated ability to self-manage, establish priorities and respond timely

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment.
