Key Responsibilities Patient Assessments and Care Planning: Performs comprehensive patient assessments and collaborates with the care team to develop and implement personalized plans of care. Referrals and Coordination: Makes referrals to other hospice care disciplines as needed and documents rationale for not doing so, ensuring seamless coordination of care. Documentation and Record-Keeping: Develops, prepares, and maintains accurate and timely patient care progress records, adhering to care center policies and procedures. Pain and Symptom Management: Records pain/symptom management changes and outcomes, ensuring effective care and support for patients. Patient and Family Education: Teaches patients, families, and caregivers essential patient care techniques, empowering them to manage care effectively. Communication and Collaboration: Regularly communicates with clinical managers and care teams to ensure cohesive care and address patient needs. On-Call Responsibilities: Performs on-call responsibilities and provides on-call services to patients and families as assigned, demonstrating flexibility and commitment to care. Additional Duties: Performs other duties as assigned, contributing to the overall success of the care team. Essential Qualifications Nursing Experience: One year of nursing experience, preferably in hospice or home health care. RN License: Current RN license, specific to the state(s) you're assigned to work. CPR Certification: Current CPR certification, ensuring you're equipped to respond to emergencies. Valid Driver's License: Valid driver's license, reliable transportation, and liability insurance, enabling you to travel to patient locations. Preferred Qualifications Hospice Experience: Previous experience in hospice care, demonstrating an understanding of the unique needs of patients and families. Case Management Experience: Experience in case management, showcasing your ability to coordinate care and services effectively. Clinical Skills: Strong clinical skills, with the ability to assess patient needs and develop effective care plans.
