Clinician / Adult Outpatient
Immediately hiring for LCSW's and LPC's looking to join a progressive team in an integrated care learning environment for Adult Outpatient Clients. This team incorporates behavioral health, primary care, pharmacy, emergency services, care coordination, unhoused assistance and day treatment. Job duties include:
Therapy
ā¢ Provides effective therapy services to patients as delineated in the treatment plan
ā¢ The intensity of services provided is consistent with the patient's acuity. The treatment plan reflects changes in acuity.
ā¢ Develops and modifies an individualized treatment plan for each assigned patient and addresses the patient's identified needs. Ensures that the treatment plan is carried out as intended. Modifies the treatment plan as feedback about the client's needs and strengths is made available.
ā¢ Conducts intakes/assessments.
Documentation
ā¢ Ensures the client records accurately reflect the treatment taking place, the strategies delineated in the treatment plan are carried out and there is a logical flow to the treatment that has taken place based on the clinical information.
ā¢ Documentation is completed according to timeliness and format delineated in the policy and procedures manuals for each type of client record entry. Documentation is written such that The Centers personnel, third party reviewers and future providers can discern the plan, course and effectiveness of treatment.
ā¢ Logs all relevant phone calls in the progress notes delineating date and purpose of call.
Coordination of Services
ā¢ Provides linkage for intra and inter agency client processes.
ā¢ Informs others involved in the treatment of client of significant events or changes in treatment.
ā¢ Presents court testimony on behalf of client when required.
ā¢ Consults with school staff, parents and others to plan and integrate appropriate services.
