Clinical Pharmacist - Oral Chemotherapy
About the position Are you passionate about providing advanced pharmaceutical care in an oncology environment? Our dynamic oncology pharmacy team is looking for an experienced Clinical Pharmacist to support our oral chemotherapy patients and work-flow. This role will work alongside other pharmacists and interact with our clinical team to enhance patient safety with prescriptions of high risk / hazardous nature. As an oral chemotherapy pharmacist you will have the ability to practice at a high level using all the skills one is trained to use with their license. Additionally, you will regularly interact with clinical teams, where recommendations of alternative products are welcomed and adjustments to existing home medications is considered. The oral chemotherapy/targeted therapy landscape is a growing space and brings great complexity where you can constantly be learning in oncology. MO and IL pharmacist license required. Responsibilities • Directs and manages workflow including oversight of technicians, interns and volunteers ensuring the accuracy of their work in accordance with departmental procedures and Federal and State regulations. • Participates in departmental meetings and on committees at the health service organization, as appropriate. • Serves as medication therapy resource for activities such as drug information, training of staff, transitions of care and Medication Therapy Services. • Evaluates medication orders for appropriateness with regard to patient condition, organ function and concomitant therapies to promote optimal patient outcomes. Documents all activities and communication as appropriate. • Recognize opportunities and offer solutions to maximize efficiency and promote safety within the medication use process.
