Clinical Nurse-Ambulatory Navigator - Oncology
About the position
The Clinical Nurse - Ambulatory Navigator plays a vital role in coordinating
patient care, developing personalized care plans, and collaborating with
interdisciplinary teams to optimize treatment delivery. The role emphasizes
patient advocacy, care management, and efficient resource utilization, ensuring
a seamless continuum of care tailored to each patient's unique needs.
Responsibilities
ā¢ Coordinate care needs for assigned patients and develop comprehensive care plans across the continuum of care.
ā¢ Partner with patients and primary physicians to create customized, needs-based care strategies.
ā¢ Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure timely delivery of tests and treatments.
ā¢ Advocate for patients to ensure their care needs are met efficiently and effectively.
ā¢ Balance clinical care with financial considerations to optimize treatment outcomes and resource utilization.
Requirements
ā¢ Associates of Nursing (ASN) required.
ā¢ Active Registered Nurse (RN) license in Indiana or Nurse Licensure Compact (NLC) RN license.
ā¢ Basic Life Support (BLS) certification through the American Heart Association (AHA).
ā¢ Additional advanced life support certifications may be required per department policies.
ā¢ Specialty certifications may be required depending on department/provision of care.
ā¢ 3-5 years of clinical work experience preferred.
ā¢ Knowledge of personal computers, word processing, and database software.
ā¢ Understanding of medical terminology, medical record requirements, regulations, and policies.
Nice-to-haves
ā¢ Bachelor of Nursing (BSN) preferred.
ā¢ Outpatient practice preferred.
ā¢ Oncology experience preferred.
