Weekend Clinical Admissions Liaison-per diem (remote)
BaneCare is seeking an experienced, enthusiastic per diem Clinical Admissions Liaison to join our weekend teams. The Clinical Admissions Liaison is responsible for assessing challenging and clinically complex patients prior to admission to ensure all clinical, fiscal and regulatory criteria are met.
The Clinical Admissions Liaison will work with existing referral sources to provide clinical assessments, answer questions, address barriers and consistently communicate the referral status with the skilled nursing center and admission team members.
Rate range is: $38-$41/hour plus a weekend/holiday differential.
Position Details
· Position is per diem weekends
· Position is mostly remote
Strongly Recommended Skills & Certifications:
· Must have experience in admissions liaison role
· RN or LPN preferred but not mandatory
· Excellent communication and interpersonal skills
· Minimum of 2 years’ experience in a Post-Acute Care Setting or Skilled Nursing Facility
· Proficient computer skills
· Familiarity with electronic referral programs such as Careport, 4Next, Epic, Pasrr portal
· Strong clinical assessment skills through review of medical records
· Detail-oriented
