Clinical Case Manager Behavioral Health - Union County

Provide compassionate and dedicated case management for members with behavioral health needs in Union County. Develop and maintain positive relationships with members, providers, and community resources to support the overall well-being of our members. Collaborate with interdisciplinary team and utilize evidence-based practices to develop individualized care plans and ensure quality care for members. Conduct assessments and evaluations to determine appropriate level of care and make referrals to necessary resources. Monitor and track member progress and make necessary adjustments to care plans as needed. Advocate for members' rights and needs, ensuring they receive appropriate services and support. Maintain accurate and timely documentation of member interactions and interventions. Participate in regular team meetings and case reviews to discuss treatment plans and identify opportunities for improvement. Stay current on industry trends and best practices in behavioral health to provide the most effective care for our members. Adhere to all company policies and procedures, as well as regulatory and ethical standards, to ensure the highest level of care for our members.
