Clinical Case Manager- Behavioral Health

Coordinate and manage the care of behavioral health members to ensure high-quality and effective treatment.
Work closely with behavioral health providers to develop and implement treatment plans for members.
Conduct thorough assessments of members' behavioral health needs and develop individualized care plans.
Monitor and track members' progress towards treatment goals and make necessary adjustments to the care plan.
Collaborate with other healthcare professionals, including physicians and therapists, to ensure coordinated care for members.
Provide education and support to members and their families on behavioral health conditions, treatment options, and resources.
Maintain accurate and timely documentation of all interactions and interventions with members.
Advocate for members' needs and ensure they receive appropriate and timely care.
Stay up-to-date on industry trends and best practices in behavioral health to continuously improve the quality of care provided.
Communicate effectively with team members, providers, and other stakeholders to ensure a seamless and efficient care experience for members.
Adhere to all regulatory and compliance guidelines and maintain confidentiality of member information.
Participate in team meetings and trainings to enhance skills and knowledge related to behavioral health.
