Clinical Care Manager - Remote | WFH

We are seeking a dedicated Clinical Care Manager to join our Long Term Support Service Community Program (LTSS CP) team. As the Clinical Care Manager, you will serve as the supervisor of our LTSS CP Supports Care Coordination team, overseeing the delivery of essential services and providing guidance to ensure the well-being of our enrollees.
Responsibilities...
Provide regular supervision for Care Coordinators and Community Support Workers, fostering a supportive environment for professional growth through group and individual sessions.
Demonstrate expertise in LTSS Supports and Care Coordination, guiding the team in delivering person-centered care plans that prioritize the voice and choices of our enrollees.
Ensure that care plans are endorsed by primary care providers and shared with all members of the enrollee's care team, promoting seamless coordination of care.
Support the engagement of assigned enrollees within specified timeframes, promoting their overall well-being and independence.
Champion the achievement of Quality Measures for the LTSS CP Supports, ensuring the highest standards of care delivery.
Facilitate the development of Consumer Advisory Boards and provider networks, fostering collaboration with ACOs, MCOs, state agencies, service providers, and community organizations.
Maintain flexibility in working hours and geographic coverage to meet the needs of the program and its participants.
Offer guidance to the care coordination team in health and wellness coaching, as well as maintaining an up-to-date listing of community resources.
Participate in 24-hour on-call coverage on a rotating basis, triaging referrals and providing timely access to needed resources.
Ensure compliance with paperwork requirements for LTSS CP Supports within established timeframes, supporting the orientation of new employees and the ongoing professional development of the team.
Assist in reporting and monitoring program performance, demonstrating commitment to our mission, values, and philosophy in all aspects of the role.
Experience Required
Minimum of 1 year of supervisory experience and 3 years of relevant professional experience.
Ability to engage and work effectively with individuals from diverse backgrounds, maintaining a nonjudgmental attitude toward families.
Willingness to work across assigned geographic regions and with the target population.
Bilingual capacity and experience in Care Coordination are desired.
Preferred Qualifications
Master's Degree or RN with behavioral health expertise.
Preferred credentials include LCSW, LICSW, LMHC, LMFT, LADC 1, Licensed Psychologist, Licensed Rehabilitation Counselor, or Registered Occupational Therapist.
