Client Services Coordinator
Lifelong is a community health organization dedicated to making health and food accessible. The Client Services Coordinator will serve as the first point of contact for the Chicken Soup Brigade, providing essential communication and support to clients facing serious medical conditions and ensuring quality care and service delivery.ResponsibilitiesStaff phone lines, answer client questions about deliveries, modify client meal categories, make changes to client delivery routes, and update client information in Food and Nutrition database or other shared internal communication toolsEnter, update, and maintain accurate, timely client data to support service delivery, reporting, compliance, and outcome trackingRespond promptly to client and provider inquiries regarding services and eligibilityServe as a liaison among clients, CSB staff, and care providers to support continuity and quality of careSchedule and complete client intakes into CSB programs, including occasional home visits for homebound individualsProcess incoming mail and send outgoing mailDistribute Welcome and Recertification Packets to new clientsSchedule nutrition assessments for clientsSupport food program operations, including food preparation and distribution, as neededPerform other duties as assignedSkillsExcellent verbal, written, and interpersonal communication skillsAttention to detail and ability to manage high-volume data and documentationCultural humility and experience working with medically vulnerable and underserved populationsAbility to work independently and collaboratively within a teamCommitment to confidentiality, compliance, and client-centered serviceProficiency in English is requiredExperience in call centers, direct customer services, or direct social servicesDemonstrated ability to engage clients or consumers from diverse communities and backgroundsFluency in Spanish, Vietnamese, Cantonese, Mandarin, Somali, Amharic or Tigrinya is preferred for this roleBenefitsComprehensive medical, dental, and vision benefitsGenerous vacation (3 weeks your first year), sick leave, and 2 personal days a year401(k) matchFlexible spending accountsLife insurance optionsLong term disabilityMass transit subsidy15 paid holidays per yearCompany OverviewLifelong is a nonprofit organization that delivers food, housing, and health services to people living with chronic illnesses.
