Responsibilities
Provide direct support to patients in their homes, ensuring their comfort and safety.
Assist with daily living activities and personal care as needed.
Communicate effectively with patients and their families to address concerns and provide updates.
Maintain accurate records of patient interactions and care provided.
Requirements
Experience in home health is preferred.
Strong interpersonal skills with a focus on customer service.
Knowledge of resident rights and advocacy for patient needs.
Basic first aid certification is a plus.
Ability to work effectively with individuals with developmental disabilities or memory care needs.
