Role Summary
We are seeking experienced and compassionate CKD/ESRD Registered Nurses to provide virtual and in-person clinical support, patient education, care coordination, and remote monitoring for patients with Stage 3–5 CKD and ESRD. This role blends clinical excellence with technology-enabled care.
Key Responsibilities
• Provide RPM/CCM/MTM and transplant readiness assessments under physician supervision
• Deliver virtual and/or in-home education for patients on hypertension, diabetes, fluid overload, and renal nutrition
• Monitor and triage remote vital signs via RPM platform
• Coordinate care with nephrologists, primary care, social workers, and transplant centers
• Document all encounters and follow-ups per Medicare and MCG guidelines
• Participate in quality improvement and protocol optimization initiatives
Qualifications
• Active and unrestricted RN license in one or more of the following states: CA, AZ, NV, NM, NY (multi-state licenses preferred)
• Minimum 3 years experience managing patients with CKD Stage 3–5 or ESRD
• Experience with dialysis care coordination, transplant referral, or home therapies (PD/HHD) preferred
• Strong clinical documentation and EHR/RPM tool skills
• Bilingual (Spanish or Vietnamese) a plus but not required
• Passion for patient-centered care and chronic disease management
