CCM Care Coordinator - Remote LPN

Your Role
• Provide monthly calls and support to patients enrolled in the CCM program.
• Utilize your nursing skills to educate patients and their families.
• Monitor patients progress and work with the physicians to ensure quality patient care.
Qualifications
• Active LPN license (compact or Vermont)
• MUST HAVE SOME OFFICE EXPERIENCE - geriatrics or primary care/internal med
• Ability to sit and talk on the phone for long periods of time
• System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
• Must live and work full time in the US - you can work from anywhere within the US but not outside.
The Pay
• The LPN pay starts at $21.85+ per hour (40hrs/week)
• This is a base pay plus a bonus position. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
