Case Manager (San Diego REMOTE)
Pay Range: $46-$47/hr • Support members with chronic/complex conditions. • Role includes phone assessments, creating care plans, coordinating with providers, and connecting members to community resources. Qualifications: • Must have active CA RN license • Extensive Case Management experience.
