Case Manager RN- Oncology or Transplant

Conduct thorough assessments and develop individualized care plans for each member. Collaborate with healthcare providers to ensure timely and appropriate treatment for members. Monitor and evaluate the effectiveness of care plans, making necessary adjustments as needed. Educate members on their condition and treatment options, as well as provide emotional support and counseling. Communicate effectively with members, their families, and healthcare providers. Maintain accurate and timely documentation of all interactions and interventions. Adhere to all regulatory and company policies and procedures. Participate in ongoing training and development opportunities to enhance clinical skills and knowledge. Demonstrate a high level of compassion and empathy towards members and their families. Serve as a patient advocate and ensure their needs and preferences are being met. Collaborate with interdisciplinary teams to coordinate and manage care for members. Stay updated on current trends and advancements in oncology or transplant care. Maintain confidentiality and adhere to HIPAA regulations.
