Serve as a compassionate advocate for members, ensuring their healthcare needs are met.
Collaborate with healthcare providers, caregivers, and other team members to develop and implement individualized care plans.
Monitor and track member progress, making adjustments to care plans as needed.
Educate members and their families on health management and preventive care.
Utilize evidence-based practices and resources to support decision-making.
Maintain accurate and timely documentation of all interactions and interventions.
Adhere to company policies, procedures, and regulatory requirements.
