As a Case Manager, you will build clinically informed relationships with adults diagnosed with SPMI and support them in achieving stability, independence, and improved quality of life through individualized recovery planning and coordinated services.

Caseloads are intentionally kept low (approximately 10–12 clients) to ensure meaningful engagement and high-quality care.

• Coordinate psychiatric, medical, housing, legal, vocational, and substance use services
• Provide support in homes and community-based settings

Your case management team includes mentors, leaders and supervisors with an open-door policy and licensed professionals who are always available to consult with you.

Additionally, you will have regular team meetings, clinical supervision with your supervisor, and ongoing behavioral health training. We provide ample training on evidence-based practices and community resources. Our approach to treatment is highly team oriented, so you always have the support of your peers and supervisors.

We welcome both early-career professionals and experienced clinicians.
