Case Management Nurse - Marin
21st Century Home Health is different from most home health agencies. We’re very intentional about how we structure our nurses’ days. We use geofencing to assign tight, localized territories, which means you’re not spending half your day commuting—you’re out in the field, seeing patients and making clinical decisions where they matter most.
We welcome both experienced home health RNs and those coming from acute or subacute settings who want more autonomy, flexibility, and deeper relationships with patients.
What you'll be doing:
Use your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
You will be:
• Performing a full assessment of the patient & family upon initial visit.
• Using assessment data to develop an advanced plan of care including rehabilitation.
• Working with patients and family one on one to achieve improvement and/or stability.
• Case manage patients care episode with interdisciplinary clinical team.
• Preparing clinical notes, orders and charting in a compliant time frame.
• Collaborating with physicians and agency staff to deliver the best care possible
• Initiating and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
What we're looking for:
• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• High level of clinical knowledge in home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
• Previous experience with IV Medication infusions highly preferred
• Proficiencies initiating intravenous (IV) therapy is a plus
