Duties • Conduct thorough virtual patient assessments focused on metabolic health, including detailed medical history, dietary habits, and physical exam components utilizing remote data. • Manage and titrate complex pharmacotherapy regimens for metabolic conditions, including GLP-1 agonists, insulin, and other weight-loss and diabetes medications. • Provide long-term chronic disease management (e.g., CPT 99490) for patients with multiple comorbidities (Metabolic Syndrome, MASLD, Diabetes, Hypertension). • Order and interpret advanced metabolic laboratory panels (e.g., liver function tests, lipid panels, HOMA-IR) and medical imaging (e.g., fibroscans). • Utilize EMR/EHR systems (e.g., [Insert Your EMR Here, e.g., Practice Better, Healthie]) for accurate documentation, prescribing, and case management. • Collaborate closely with the Registered Dietitian and Care Coordinator on multidisciplinary patient treatment plans, focusing on diet, lifestyle, and personalized health coaching. • Ensure compliance with all state and federal regulations regarding telehealth, prescription monitoring, and chronic care management billing (e.g., CPT coding, ICD-10). • Engage in continuous specialized training on the latest evidence-based protocols for MASLD/MASH reversal and obesity medicine. Requirements • Valid Nurse Practitioner license with current, unrestricted Texas state certification. • Strong clinical knowledge of physiology, pharmacology, and anatomy related to the Endocrine, Hepatic, and Gastrointestinal systems. • Proven proficiency with EMR/EHR systems; familiarity with telehealth platforms is highly desirable. • Experience in metabolic health, weight management, endocrinology, or gastroenterology is preferred, but strong candidates with a passion for specialization will be considered. • Commitment to the long-term, relationship-based care inherent in chronic disease management, rather than episodic urgent care. • Ability to work independently in a 100% virtual environment, demonstrating exceptional organization and communication skills. • Must possess a valid DEA registration or be eligible to obtain one.
