We are currently hiring a Care Support
Summary: Provides non-clinical support to Care Managers by providing administrative and logistics support for Care Manager’s member caseload in the Care Management continuum. Performs administrative tasks for development and implementation of care plans, including outreach to providers and community resources to connect member and to perform appropriate follow-up.
Location: 100% Remote, candidates MUST reside in California
Required Skills:
• Data Entry experience
• Customer service experience
• Administration background
