This includes working closely with members to develop ongoing care plans, identifying necessary providers, specialists, and community resources. The Care Navigator will also provide psychosocial support and resource assistance to members and caregivers, helping them access local services related to employment, education, housing, and more. Coordination is key in this position, as the Care Navigator will liaise between members, their families, and the care provider team to ensure timely access to identified services. Monitoring progress towards care plan goals and collaborating with healthcare providers for necessary revisions will also be part of the responsibilities. The role may involve on-site visits to assess member needs and provide education on healthcare options and benefits. Responsibilities

Requirements
? Bachelor's degree in a related field.

? 2 - 4 years of related experience in healthcare or care management.

? Graduate from an Accredited School of Nursing if holding clinical licensure.

? Current state's clinical license preferred.
