Care Manager – Social Worker (Older Adults, Remote)

Hera provides a dedicated care manager in the community, covered by Medicare, to be the trusted guide to age at home. We help the family just be family again. We’re looking for mission-driven social workers or nurses, ideally with experience in hospice, palliative care, or geriatrics, who are passionate about making a difference in the lives of older adults and their families. This is a flexible, remote, 1099 opportunity with the path to grow into a leadership role on our team.
