In this RN Care Manager role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. Expect to spend about 50% of your time visiting our members in their homes or in long-term care facilities. You'll need to be flexible, adaptable and, above all, patient in all types of situations. We offer our established staff the option to work 4 10-hour days (7 am-6 pm) in lieu of the traditional 8 hour 5/day week schedule. Additionally, there is no 'on-call' or weekend requirements. Responsibilities • Assess, plan and implement care management interventions that are individualized for each member and directed toward the most appropriate, least restrictive level of care • Identify and initiate referrals for both healthcare and community-based services; including but not limited to financial, psychosocial, community and state supportive services • Develop and implement care plan interventions throughout the continuum of care as a single point of contact • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members • Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team Requirements • Unrestricted RN license in the state of North Carolina • 2+ years of RN experience • Intermediate level of proficiency with Microsoft Office Suite • Resident of areas within Cabarrus/Concorde/Kannapolis, Stanley, Union, Anson, Albemarle, Monroe and Wadesboro, NC • Willing or ability to travel, up to 50% of your time to visit Medicaid members in their homes and/or other settings, including community centers or providers' offices • Driver's License and access to a reliable transportation Nice-to-haves • Bachelor of Science in Nursing (BSN) • CCM certification • 2+ years of experience working within the community health setting or 2+ years of experience providing community-based care management to members receiving long-term care, personal care services, private duty nursing, or home health • Experience working with long term services and support, to complete personal care service assessments • Experience in managing populations with complex medical or behavioral needs • Experience working with Home Care Based Services • Experience working with members in community and home-based settings • Fluent in Spanish Benefits • Comprehensive benefits package • Incentive and recognition programs • Equity stock purchase • 401k contribution Apply tot his job
